Create a Custom Form

Before creating a new form, make sure that you have a Category created. This is required in order to save your form. Creating a new form is a three step process.

To create your form:

  1. Click Custom Form Manager on the right
  2. Click the Add a Form tab at the top of the page
  3. Enter at least the required information (name and category)
  4. Submit the form to go to the Sections page
  5. If you would like your form to be broken into sections, add those sections here
  6. Submit the form to add fields
  7. Add the fields that you would like in your form - each field can have a label, type, options (if applicable), an doutput label, active, required, display order, and a "handle" for use in design integration.
  8. Submit the form

Your form is now created, and is ready to be assigned to a page.
